I owe a debt of gratitude to many people who, in different ways, have helped me to complete this second edition.
At my university, I wish to thank Professor Jack Anderson, Professor Brice Dickson, Professor Kieran McEvoy, Professor John Morison, and Professor Sally Wheeler.
Jack and Brice, in particular, read chapters and sections for me, and pointed me in the direction of some cases that I had missed.
