There are a few people we would like to thank. Of course we thank the authors for writing the chapters, but also for being willing to strictly follow the structure of each chapter, which we imposed on them so as to facilitate comparison between the different chapters. We also thank the peer reviewer who made very useful comments which contributed to improving the book. Finally we thank Gary Bell's research assistant, Ms Mingtse Chen Ouw, who helped tremendously in the editing of the chapters, particularly the footnotes and bibliographies.
